New Students to River Trail Middle School
REGISTRATION REQUIREMENTS
- Affidavit of Residence and Proof of Residency – Affidavit of Residence must be notarized and signed by Parent/Guardian and Homeowner/Apartment Lessee. Two documents indicating residence in the attendance area must be provided. Appropriate records for verification of residency must include any two of the following: home mortgage payment book, current utility bill, apartment lease showing name of legal guardian, homeowner’s or renter’s insurance registration/card, current paycheck stub, current bank statement, sales contract or receipt to have utilities connected.
- Withdrawal Form from previous school listing classes and grades at time student left.
- US Government issued Birth Certificate - we will make a copy and return original to you.
- Social Security Card or signed waiver – we will make a copy and return the original to you.
- Georgia Certificate of Immunization Form 3231 or signed waiver*. Entering 6th graders must show proof of two (2) doses of Measles vaccine, two (2) doses of Mumps vaccine, and one (1) dose of Rubella vaccine or laboratory proof of immunity against Measles, Mumps or Rubella AND show proof of two (2) doses of Varicella (chicken pox) vaccine or documentation of disease or laboratory proof of immunity. These certificates are available from the local health department or private physician.
- Georgia Certificate of Ear, Eye, and Dental Screening Form 3300 or signed waiver. This certificate is required for students entering a Georgia school for the first time. This form is available through the local health department or private physician.
- Complete mailing address, fax and telephone numbers of last school attended.
- Copies of recent standardized test scores (if available). This information is necessary for placement into classes as well as qualifying for foreign language classes open to grades 7 & 8.
- A copy of Individual Education Plan (for students receiving special education services), eligibility reports, psychological test reports and/or qualifying test scores that were used if child is enrolled in any special programs (special education, talented and gifted program, etc.).
- Specific written documentation from the student’s former school confirming recommended placement in any advanced class. It is the parent’s responsibility to provide this information.
- Proof of guardianship (if applicable).
- “signed waiver” is accepted during the school year only.
